MUST BE RECEIVED BY JANUARY 16, 2008

All applicants must submit the Doris Duke CRF Common Application package to UNC-CH by the date listed above. The total application package for UNCincludes the following:

• Email confirmation of CRF Application Form submitted directly to the Foundation
• Transcript of your medical school record
• A letter of support from the Dean's office of the medical school in which you are currently matriculated.
• Two letters of support from Faculty who can comment on your academic performance and potential for clinical research
